Doctor: How many days has it been to your surgery?

Patient: It has been 15 days to my Bariatric surgery. Doctor: Ok. Patient: Yes! Doctor: What major problems you had? Patient: I had severe breathlessness. Doctor: Ok. Patient: I used to sleep while sitting. Patient: I used to fear that I might sleep while driving. Patient: It was difficult to do slight movements of body, also, I wasn't able to stand for long times. Patient: I used to feel stress on my chest, whenever I walk. But after Bariatric surgery, I feel light. Doctor: You had Diabetes before surgery and you were on medication. Patient: Yes! I had Borderline Diabetes Mellitus and I was on medication. Doctor: How many tablets were you taking? Patient: I used to take medication twice a day. Doctor: How you felt in these 15 days after surgery? Doctor: How much are your blood sugar levels? Patient: In these 15 days my blood sugar levels varies around 110-130mg/dL, without any medication. Doctor: Ok. Patient: Yes! Doctor: Today we will discuss about your heart problem, you were suffering from Dilated Cardiomyopathy. Patient: Yes! Doctor: You gained weight. How much was your weight at the time of surgery? 176 Kg? Patient: Yes! Doctor: The load on your heart increased as compared to body weight. Patient: Ok. Doctor: Because of overweight and some other issues such as high blood sugar levels, alcohol intake and high blood pressure, your heart muscles became weaker. Doctor: There are two types of balloons, thick and thin. Heart muscles are like a thick balloon that they can't burst easily. Doctor: Dilated cardiomyopathy is a condition in which the heart muscle becomes weakened and enlarged. Doctor: As a result, the heart cannot pump enough blood to the rest of the body. Patient: After surgery, I feel very light. There is no stress on heart and mind. I feel very relaxed. Doctor: In critical patients (Heart patients) like you, our main focus is to complete the surgery in short duration. Doctor: In Mini Gastric Bypass surgery, we give TIVA anesthesia, for very short duration. Doctor: With TIVA anesthesia, you will go in deep sleep. This is a switch on and switch off type of anesthesia. Doctor: When we switch off it, you will immediately wake up. Patient: Ok. Doctor: Secondly, we want early recovery in heart patients and we don't want to insert any tubing's such as NG tube. Doctor: Also we want you to start taking liquids and semi-solids on the 2nd or 3rd day of surgery. Doctor: If you start taking proper meals then you will feel this surgery as a minor one. Patient: Yes! Doctor: We have changed all the connections from inside and you will lose good amount of weight. Patient: Yes! Doctor: And this weight loss will lower your heart overload. Doctor: In patients like you, MGB becomes simpler as it is completed in short duration. Doctor: Also we give TIVA anesthesia, for short duration. Patient: Ok. Doctor: Post-operative recovery is very fast. Doctor: You had Sleep apnea, were you using CPAP machine before surgery? Patient: Yes! I am still using that. Doctor: Continue it for next 15 days. Patient: Ok. Doctor: Stop using it when you will lose more 10kgs and your neck becomes thin. Patient: Ok. Doctor: Today, we will start your multivitamins, take them regularly. Patient: Ok, I will take that. I will follow your instructions. Doctor: Start eating solids from today onwards, as we have advised. Patient: Ok. Doctor: Don't eat solids very fast. Patient: Can I take fruits now? Doctor: Yes! Start from today, but take 1-2 pieces. Patient: Ok. I feel full with 1-2 pieces. Doctor: Every person is different. Patient: Yes! I feel full with 1 bowl of soup. Doctor: Have you ever had vomit after taking anything till now? Patient: No, never. I have no nausea or vomiting.
