What is Sleep Apnoea?? What is the role of MGB Surgery in Obstructive Sleep Apnea?
Doctor: Mr. Jang Bahadur, tell us about Obstructive sleep apnea and snoring which you had before surgery?Doctor: What is the difference in it after surgery and how was it before? Patient: I remember that before MGB, I had never slept comfortably in the last 4-5 years. Doctor: Ok. Patient: I used to sleep while sitting. Doctor: You could not even lie down. Patient: No, I used to lie down but when I woke up, I find myself sitting. Patient: I didn't know when I woke up. Doctor: Ok. Patient: I used to sleep while sitting and my whole night was spent like this. Doctor: Ok. Doctor: How much time did it take to cure this and how did you got rid of it? Patient: My surgery was done on December 3rd and today is January 13th. Patient: I started to sleep in a lying position after 15 days of Bariatric surgery. Do you sleep comfortably now? Patient: Yes! I don't use a Bi-PAP machine now. Ok. Patient: Yes! Doctor: Here I would like to explain, what happens with a heavy neck? Doctor: Because of fatty tissues around the neck, the tongue falls, which causes obstruction in the airway. Patient: Ok. Doctor: This is the main problem which is a very common cause of death among youngsters these days. Doctor: Many famous players die while sleeping. Doctor: Sleep apnea is the main problem in the majority of cases where people die because of breathlessness while sleeping. Doctor: Apnea means the absence of breathing and anyone can check this by recording a video of them while sleeping. Doctor: You can see a sudden loud gasp or jerk to your body while sleeping and again you will snore regularly. Patient: Yes! Doctor: At that moment, your oxygen levels drop. Patient: Ok. Doctor: Oxygen deficiency is because of the interrupted exchange of gases, caused by tongue fall. Patient: Ok. Doctor: Retention of carbon dioxide in the body cause daytime sleepiness and laziness. Patient: Yes! I used to sleep while sitting. Doctor: This causes bluish discoloration of the face. Doctor: Have you noticed any complexion changes on your face after surgery? Patient: You won't believe that I can remove body dirt after surgery. Doctor: That body dirt was because of carbon dioxide retention in the body. Like if you put rubber on your finger it will become bluish. Your fat on the neck was like that rubber only. Patient: Ok. Doctor: We can see the difference in color complexion by comparing your before and after surgery pics. Patient: Yes! My complexion has improved as compared to before surgery. Patient: Obese patients have darkness around their necks. Doctor: Yes! Doctor: Everything is connected to obesity. Patient: Ok. Doctor: We need to tell everyone about Sleep Apnea and make people aware of it. Patient: Yes! Doctor: Because people used to fear surgery whereas they should have fear of sleeping with a heavy neck. This causes obstruction and a drop in oxygen levels at night. Doctor: At least people must be aware of the CPAP machine and they should use it. Patient: Yes! Doctor: CPAP is a constant positive airway pressure machine that treats sleep apnea by delivering a stream of oxygenated air into your airways through a mask and a tube. Patient: Ok. Doctor: As you sleep, the airstream from the CPAP machine pushes against any blockages, opening your airways so your lungs receive plenty of oxygen. Patient: Ok. Doctor: We recommend patients use a CPAP machine for at least one month after surgery. Doctor: How much weight have you lost this month? Patient: At the time of surgery my weight was 149 Kg and today my weight is 135 Kg. Doctor: Ok, you have lost 14 kg in a month and you feel good after losing this much weight. Patient: Yes! Doctor: But still, you needed a CPAP machine this month. Patient: Yes! Doctor: Now, you don't need that. Patient: Yes! Doctor: You will lose more weight gradually. Patient: Ok. Doctor: Keep taking multivitamins regularly. Patient: Yes! I am taking that. Doctor: Start doing some exercise. Patient: Ok. Doctor: You need to come for follow-up after 3 months. Patient: Ok.
