What is Dumping?? Is it Good or Bad ?? How to manage ??

Doctor: You got Bariatric surgery done 2.5 years ago and after that, you have lost almost 60 kg weight.

Doctor: You are saying that sometimes your blood sugar levels go below the normal range. Patient: Yes! Doctor: Then, do you eat something sweet? Patient: Yes! Doctor: Even after taking anything sweet, it keeps on decreasing. Patient: Yes! Doctor: Ok. Doctor: This is called Dumping Syndrome. Doctor: You need to understand how to handle Dumping syndrome. Doctor: This is an indication that your surgery is working. Patient: Ok. Doctor: As we all hear Mini Gastric Bypass surgery helps to control high blood sugar levels. Patient: Yes! Doctor: How does MGB control high blood sugar levels? Doctor: Now, whatever you are eating, is touching a new point on the intestine because of bypass. Patient: Ok. Doctor: Like, before surgery food used to touch the first part of the intestine, but after surgery food touches the new bypass point on the intestine. Doctor: In your case, the length of the bypass is 150cm (approx 5 feet) from the total of 28 feet, which is a normal length. Doctor: After bypassing 5 feet, 23 feet intestine is still there. Patient: Ok. Doctor: When your food especially sugars and carbohydrates (chapatis and rice) touches on the bypass point. Doctor: This new point has never seen direct sugars and carbs before, so this is quite sensitive towards sugars and carbs. Doctor: Like underarm area is more sensitive than the chest area. Doctor: Like this, further parts of the intestine are sensitive towards sugars and carbs. Doctor: If you eat carbs (like parathas or chapatis) in the morning and take tea along with that. Doctor: Then there are chances that you will feel uneasiness and perspiration after 30-35 minutes. Doctor: If you will check RBS at that time it will be around 60-65 mg/dL, and we get scared at that time. Doctor: Understand that whenever you will eat carbs or sugars, the new sensor (bypass point) sends strong signals to the pancreas to produce more insulin to control increased glucose levels. Doctor: This Insulin lowers Blood sugar levels. Doctor: The girl who was sitting here was taking Insulin before surgery and it has been only 1.5 months to her surgery and she doesn't need Insulin now. This is the reason behind that. Doctor: Because you got a new sensor now (bypass point). Doctor: But this can also drop your Blood sugar levels below the normal range, and you can feel uneasy, perspiration, and low energy levels. Doctor: How to avoid this? Doctor: Firstly whenever you feel like this, eat Hajmola candy. Always keep Hajmola candies with you. Doctor: Don't eat sweet candies as this will lower your Blood sugar levels again. Patient: Ok. Doctor: Better would be to eat any seasonal fruit with salt. Patient: I do this. Doctor: You will feel good after 5 minutes. Patient: Yes! Doctor: But we want to avoid Dumping. Doctor: To avoid this, don't eat or drink anything sweet. Doctor: This will help you to avoid dumping and also to maintain weight in the long run. Doctor: Eating too much carbohydrates such as chapatis, poha or rice can lead to dumping. Doctor: To avoid this your meal should consist of 80% of healthy things such as curd, vegetables or salads and carbs should be 20% only. Doctor: Sometimes we make the mistake we eat 1-1.5 chapatis and a small amount of lentil. And we feel full with that. This meal is 80% carbs and 20% healthy things. Doctor: But we want 80% of healthy things such as curd, vegetables or salads and carbs should be 20%. Patient: Ok. Doctor: Better would be to eat a salad before the meal. That contains a good amount of fiber. Doctor: You can eat anything seasonal in the salad. Patient: Yes! I eat onions and cucumbers. Doctor: The other reason for dumping can be long gaps in the meals. Doctor: Like if someone got this surgery done, and he had eaten little in the breakfast and then nothing after that can have dumping syndrome. Doctor: You must have noticed that your body wants something to eat after short intervals. Patient: Yes! Doctor: Your body needs something to eat after short intervals, to eat anything. Doctor: Like while going anywhere or while working in the office. Patient: I eat nuts or salty biscuits whenever I feel like eating. Doctor: Don't eat biscuits, eat fruits. Patient: Ok. Doctor: After eating any fruit you won't feel hungry for some time and also you will not have dumping. Patient: Ok.
