Doctor: We have done your surgery for Diabetes?Patient: Yes! Doctor: We call it Metabolic Surgery. The technique is almost the same, we make some customizations such as a bigger pouch and longer bypass to decrease absorption. In your case, we didn't want you to lose more weight. Patient: Yes! Doctor: How much was your weight at the time of surgery? Was it 84 Kg? Patient: Yes! Doctor: Today your weight is 72 Kg. Patient: Yes! Doctor: You have lost 12 Kg. Patient: Yes! Doctor: It has been more than 1 year to your surgery. Patient: Yes! Doctor: Is your weight stable now? Patient: Yes! Ok. Doctor: Can you eat like a normal person? Patient: Yes! Doctor: Can you eat 2 chapatis with vegetables or pulses? Patient: No, I don't eat chapati. But I eat chicken with vegetable or pulses like a normal person. Doctor: How are your blood sugar levels now and before surgery? Patient: Before surgery, my blood sugar levels were more than 200-250mg/dL even after taking Insulin. Ok. Doctor: How are your blood sugar levels now? Patient: Now, my Fasting blood sugar levels used to stay in between 70-100mg/dL. Ok. Patient: In the evening, my blood sugar levels vary from 112-115 mg/dL. Ok. Doctor: Did you inject Insulin after surgery? Patient: No. Doctor: Do you take any tablet now? Patient: No, I don't need anything. Doctor: Have you discussed with your GP that you got this surgery done? Patient: She wasn't happy with my decision. Doctor: Because she told you that you don't need this surgery? Have you consulted with her now? Patient: Yes! I met her after surgery. Firstly she thought.... Then she checked everything. She called me after one day and said.. Doctor: The main concept of Metabolic surgery is very simple. Why did you get Diabetes? You were not diabetic since childhood? There may be some variations happened which caused you diabetic. Patient: Yes! Doctor: What happened? Why were you Diabetic? It was because of Insulin resistance. Your body was making Insulin but your body became resistant to it. You had to get Insulin injections. Your weight was not that much. Patient: Yes! Doctor: 84 Kg is not too much weight. You had just 12-15 Kg extra weight. Now, the food is touching a new point in the intestine (bypass point). This new point works as a new sensor because it has never seen any food directly. Now, food is touching directly on this new point. And this sends stronger signals to the Pancreas. Earlier Pancreas was in sleeping mode. Now, pancreas in getting stronger signals to make Insulin. We are habitual of eating carbohydrates daily and because of this your sensor was desensitized. It was not sending proper signal to the pancreas. Your Pancreas was healthy but was in sleeping mode. This is called Type II Diabetes. Now, Pancreas is receiving high amplitude signals. Sometimes after drinking sweet tea or coffee, you will feel uneasy. Patient: Yes! Blood sugar levels go down after this. Doctor: Yes! this is called rebound hypoglycemia. This new (Bypass) point has never seen sugars and carbohydrates before surgery. Earlier, it goes to the stomach first and then to the first part of the intestine. When it reaches this point sugar was already absorbed in the blood. Like your GP has told you that.. You are still potentially diabetic. Your diabetes will reoccur if you gain weight. You said you don't like to eat chapatis, which is very good thing. If you start eating carbohydrates daily then your new sensor will also desensitize in the future. It is really easy to understand this. Your Hba1c before surgery was 13.9, which is 6.3 now. Patient: Yes! Doctor: It has been 1.5 months to your surgery. Patient: Yes! Doctor: Since how long have you been taking Insulin? Patient: It has been 4 years since I was taking Insulin. Ok. Doctor: How old your Diabetes was? You are diabetic since 2003? Patient: Yes! Doctor: You were diabetic for a very long time. Patient: Yes! Doctor: You didn't need Insulin after surgery? Patient: No, I don't need Insulin now. Before surgery, I was taking Anti-glycemic tablets and I don't need that too. Doctor: You are not taking Insulin or tablet. Do you check your blood sugar levels daily? Patient: Yes! Doctor: At the time of surgery your weight was 85 Kg and you lost 8 Kg till now. Patient: Yes! Doctor: With more weight loss your Diabetes will become better. Understand this, with this surgery we didn't cured Diabetes. Your blood sugar levels are under control. Get your blood tests done regularly. Get your Hba1c checked every 3 months. Patient: Ok.
